As a Nurse working with TLC Care, you will play a vital role in ensuring the physical, emotional and social well-being of residents within our care home. This position requires you to lead and guide the care team, ensuring that every resident’s needs are met comprehensively. You’ll assess, develop and evaluate tailored care plans for each individual, maintaining close collaboration with residents, general practitioners (GPs), family members and other caregivers.

Main Purpose

Care Delivery and Planning
- Monitor residents’ health, reporting any issues to relevant professionals and arranging necessary GP or specialist visits.
- Ensure that each resident’s care plan is personalized, up-to-date and created in alignment with NMC guidelines and company policies.
- Conduct health assessments for new residents and ongoing reviews for existing residents.
- Provide direct nursing care and health supervision as required.
- Administer prescribed medications accurately and document them following NMC regulations.
- Serve as the named Nurse for a designated group of residents.
Tissue Viability
- Prevent care home-acquired pressure sores by delivering top-quality care.
- Document and report any pressure sores to relevant parties.
- Develop and implement care plans for residents identified as being at risk of pressure sores.
Nutrition Management
- Complete monthly MUST (Malnutrition Universal Screening Tool) assessments for residents.
- Create care plans for individuals at nutritional risk, detailing interventions and support.
- Maintain records of residents with special dietary needs and communicate these to the kitchen staff.
- Oversee care plans for residents requiring PEG feeding (Percutaneous Endoscopic Gastrostomy).
Continence Care
- Conduct continence assessments for residents as needed.
- Ensure all residents with catheters have detailed care plans outlining specific catheter care.
Infection Control
- Promote and adhere to infection control policies within the home.
- Monitor staff to ensure compliance with universal precautions including handwashing and appropriate use of gloves and aprons.
Hygiene and Grooming
- Ensure residents are well-groomed, appropriately dressed and that their personal choices are respected in care documentation.
Mobility Support
- Facilitate residents’ independence by ensuring access to mobility aids and encouraging safe movement.
- Verify that bedrails are only used when necessary and appropriate.
- Develop care plans to minimize fall risks for residents at higher risk of falling.
Dignity and Respect
- Foster an environment of respect where residents are addressed by their preferred names and treated courteously.
- Monitor staff interactions to ensure residents are engaged personally and respectfully.
Communication
- Attend and actively participate in staff, resident and family meetings as required.
- Establish strong communication channels with residents, families and multidisciplinary teams.
